Services is requesting a total of $10,480.00 from the El Pomar Foundation to enhance the
Home Connections Program and the Volunteer Services Medical Transportation
Pro~nain. Both services support independence, engaging volunteers to help others in the
coinmunity and keeping our older adults safely in our county. This financial assistance

For three decades, Eagle County Government in collaboration with Alpine Area on
Aging has served the needs of the community with a full compliment of programs to
support healthy, independent living for older adults.
Prior to the late 1970's, senior services in Eagle County consisted of programs and
activities that existed ahnost exclusively through the efforts of grass-roots volunteerism.
In 1979, a proposal was made on behalf of the seniors of Eagle County to the Board of
County Commissioners to fund the Elderly Services and Needs Program. In 2006, Adult
Services became an official division of Eagle County Health and Human Services. With
this change, programs that were once piecemealed throughout Eagle County including
senior nutrition, senior health activities, adult protective services and volunteer services
were incorporated into one inclusive division.
Today, Eagle County Government remains the main source of services for older adults in
the county. Eagle County Aging and Adult Services (renamed in 2008) provides
leadership to create and sustain an aging-well cominunity that promotes social and civic
engagement, planning along the life span, optimizes physical and mental independence,
supports community caregivers and addresses the basic needs and safety concerns of
older adults and persons with disabilities. Eagle County Aging and Adult Services
includes programs for nutritious lunches, health awareness and disease prevention
activities, and basic transportation services throughout the week at senior sites in Basalt,
Eagle and Minturn. Eagle County Government also offers options for independent senior
housing, caregiver support groups, vision support groups, and social/ educational
activities. To address both adult protection requirements and the need for resource
coordination, the case manager investigates abuse, neglect, and exploitation of seniors
and disabled adults in the comrnunity and coordinates services to ineet the needs of
at-risk adults who strive to reinain independent and safe in their hoines. Eagle County
Volunteer Services rounds out the Aging and Adult Services Departinent by supporting
older adults with options for volunteering in the community through the Rural Resort
RSVP Program. Volunteer Services also recruits and organizes a network of volunteers
of all ages to assist with many aspects of Aging and Adult Services.
Eagle County Aging and Adult Services, in it's short time as a single department, has
made significant progress in reaching out to older adults in the community, streainlining
services and developing a strategic plan to continue meeting the needs of older adults in
the community. Eagle County Aging and Adult Services involves over 120 older adults a
year in the nutrition sites and health activities. In addition, the case manager serves 60
at-risk older adults a year.
In 2009, Eagle County Aging and Adult Services began new efforts to provide a stronger
system of care for older adults in Eagle County. In collaboration with the United Way of
the Eagle River Valley, Aging and Adult Services launched Home Connections which
provides free housekeeping services to older adults who are unable due to injury or frailty
to do housekeeping chores independently. In addition, Eagle County Health and Human
Services hopes to begin a small early dernentia day care pro~-am for older adults with
Alzheimer's disease in collaboration with the Brookdale Foundation. Eagle County
Health and Human Services submitted a grant application to the Brookdale Foundation
for this new service in June. Eagle County is in the planning stage of developing a small
assisted living facility as well. All three of these programs would be the first and only
service of their kind in our county.
Eagle County Volunteer Services supports the efforts of Aging and Adult Services by
recruiting volunteers who serve as companions for older adults or who assist with
running errands, preparing and delivering meals and providing transportation. Eagle
County Volunteer Services coordinates a medical transportation pro~nam in which
volunteers provide rides for individuals to essential medical appointinents. In 2008, Eagle
County volunteers drove individuals over 10,672 miles for a total of 123 rides to medical
appointments.
It is an exciting time of strategic planning and growth for Aging and Adult Services. Our
division is ready to expand current programs and launch new services to better meet the
needs of our community.
Purpose of the Request
Eagle County Aging and Adult Services is requesting a total of $10,480.00 froin the E1
Pomar Foundation to enhance two essential services that support safe and independent
living for older adults in Eagle County; The Eagle County Home Connections Prograin
and the Volunteer Services Medical Transportation Program. Both programs support
independence, engaging volunteers to help others in the community and keeping our
older adults safely in our county. With the current economic times as they are, these
supports are more ilnportant than ever in that they help to prevent accidents and/or
injuries of older adults in their hoines or cars anci placement in a more expensive, higher
level facility. These programs also assist the caregivers and fainily meinbers of older
adults who often struggle to care for their loved ones in the absence of financial and
programmatic resources.
Home Connections
Eagle County Aging and Adult Services is requesting $6480.00 for Hoine Connections.
Eagle County Hoine Connections provides free housekeeping to local residents who are
at risk of losing their independence due to not being able to keep their living
environments safe and clean. It is estimated that a minimuin of fifty adults aged sixty
years and older in Eagle County are being underserved and are in at-risk living situations
due to the absence of community, non-medical, in-home services, including
housekeeping and home maintenance assistance and non-skilled nursing for bathing and
meal preparation. Our local home health agency supplies skilled nursing services only
and there are no non-skilled nursing, adult day care and respite care agencies in Eagle
County. Often our community residents aged 60 years and older are forced to inove out
of their communities to receive needed assistance or a higher level of care or they stay at
lloine putting theinselves at risk of accident, injury or illness. At least ten of those
individuals are at imminent risk of institutionalization.
Eagle County Home Connections aims to fill this gap in the community by providing free
housekeeping two times a inonth to local residents who are aged 60 years and older, who
live at or below 250% of poverty and who demonstrate physical or mental incapacities
that require intervention to remain safely in their home. Eagle County Home Connections
contracts with a local housekeeping proprietor who provides the housekeeping service for
$20.00/hour. It is a goal of Home Connections to prevent accidents and injuries of the
older adults, as well as prevent the involvement of more costly adult protection services
and the entry into an assisted living facility
In 2009, Home Connections provided housekeeping help to seven different older adults.
These residents range in age from 69 to 87 years and live throughout the county. Five of
the individuals are males and two are females. Five of the individuals use assistance for
walking including a cane, walker and/or wheelchair. All of them live independently and
have liinited fainily support. Here are a couple of examples of the older adults and their
current situations. Older adult #1 is a female, aged 79 years, living in Eagle and using a
walker and wheelchair due to stenosis of the spine and lupus. Older adult #2 is a inale,
aged 69 years, living in Eagle who has difficulty walking and using the right side of his
body due to a recent stroke, and older adult #3 is a male, aged 79 years, living in Eagle
who has kidney failure and has to travel out of the county an average of three days a
week for dialysis. For each of these individuals, the house cleaning proprietor vacuums
the floors, cleans the bathtub and/or shower, toilet and sink and mops the floors, which

due to mobility problems and poor health, these clients cannot manage on their own.
Ho~ne Connections has met an essential need for each of these individuals.
Eagle County Aging and Adult Services marketed Home Connections on a small level at
the senior meal sites in Minturn and Eagle. At this time there are five additional adults on
the waitlist to receive Home Connections services. In addition, Eagle County has not
begun to expand this program in the El Jebel-Basalt area where there are an estimated
five residents in need of this assistance. Aging and Adult Services aims to support ten
clients with housekeeping help in the next year and expand Home Connections to include
twenty local residents within the next two years.
Our request of $6,480.00 will provide the housekeeping services for six older adults for
twelve months. Eagle County Home Connections is also funded by the United Way of the
Eagle River Valley and Eagle County Government. This combined funding will allow
Eagle County to serve a total of ten clients in 2010.
Medical Transportation:
Eagle County Aging and Adult Services is also requesting $4000.00 for the Volunteer
Services Medical Transportation Program. The medical transportation program is a vital
service in Eagle County focused on supporting older adult independence and community
helping community. Volunteer Services receives calls from both individuals themselves
and community professionals on the behalf of individuals in need of the medical
transportation assistance. Once eligibility and the details for the appointment are
established, Volunteer Services contacts a network of registered volunteers to determine
who might be available to help with the request. Eagle County Volunteer Services
consistently recruits new volunteers for this ongoing opportunity. Volunteer Services
receives as many as 150 requests annually for transportation to essential medical
appointments both within and outside of Eagle County and is able to match 80% of these
needs.
There are few supportive services for transportation in the county. Eagle County in
collaboration with the Alpine Area on Aging provides transportation for local older adults
on the Senior Center buses. The bus drivers coordinate group transportation to medical
facilities as well as grocery stores, post offices, pharmacies, retail shops, and for social
field trips. Within this service, medical transportation is limited especially to appointments
outside of the county area. The other option for older adults is ECO Para-Transit which
offers transportation for adults aged 60 years or older or individuals with disabilities who
complete an application and meet eligibility requirements. There are difficulties with this
service because the bus will only stop at locations no more than '/ of a mile from a bus
stop. In addition, there is limited door to door service and individuals are expected to be
present 30 minutes before the scheduled pick up time. These programs do not meet the
need for all medical appointments nor do they support independence and flexibility. Due to
the lack of resources and the increase in the aging population, the demand for volunteer
provided medical transportation continues to increase.
In 2008, Volunteer Services expended $3,587 to volunteers for travel reimbursement for
over 10,672 miles. There were also several other volunteers who provided rides who
declined reiinbursement for their travel and instead donated both their time and their gas
expense. For the first six months of 2009, Volunteer Services expended $4553.68 in
mileage reimbursement for medical transportation; over 11,000 miles have been logged
for a total of 63 individuals. A dramatic shift in utilization is happening such that inany
more out of county trips are being requested. This is reflective of the increase in our
aging population and their medical needs that cannot be met locally. About a third of the
rides are the same clients requesting recurring services/trips. Most medical specialists
only have offices in the larger cities of Glenwood Springs, Grand Junction, or Denver.
Further, our trends show that there are fewer requests for medical transportation during
the suinmer months when clients typically drive themselves especially in County.
However, during winter months, many of our seniors will not drive in inclement weather
conditions which are typical to our area.
Over 20 volunteers generously donate their time to assist people in getting to their
medical appointments. Eagle County encourages their volunteering through reiinbursing
$ 0.40 a mile. Some volunteers do often choose not to be reimbursed for their travel
however inany need the reimbursement in order to continue helping especially during the
current economic times.
The request of $4000.00 for the medical transportation program will reimburse volunteers
for 10,000 miles. Eagle County Government will budget $4000.00 as well to reimburse
volunteers for a total of 20,000 miles in 2010.

Eagle County Aging and Adult Services launched Home Connections in April 2009 and
thus the current budget allows for 9 months instead of 12 months. Eagle County plans to
provide the Home Connections service for all of 2010. The United Way of the Eagle
River Valley awarded Aging and Adult Services $1000.00 for Home Connections for
2010. In addition, Eagle County is budgeting an additional $3500.00 for Home
Connections in 2010. Aging and Adult Services plans to continue the program for at
minimum 4 clients if we are not able to receive El Pomar funding and to continue
searching for grant opportunities that can support this vital program.
Eagle County originally budgeted $3800.00 for Volunteer Services medical
transportation in 2009. In May 2009, Eagle County amended the budget to include an
additional $6200.00 to meet the growing need. Eagle County is currently studying
options for handling the increase in demand for medical transportation and to enhance the
system of transportation in Eagle County. One new goal is to regularly include reviewing
eligibility for Medicaid reimbursement for clients who receive the service so that
volunteers can be reimbursed from Medicaid rather than Eagle County.

Additional Financial Aid Assistance
Eagle County has requested financial aid from the United Way of the Eagle River Valley
for the last two years. In 2008, Eagle County requested $5450.00 and received $2500.00
for Home Connections. In 2009, Eagle County requested $8000.00 from the United Way
and was awarded $2000.00. Eagle County Health and Human Services has never
requested additional outside funding for the volunteer medical transportation prograin
however due to the recent increase in demand for the service, Eagle County is pursuing
outside support.

Our Governing Board, the Eagle County Board of County Commissioners, consists of
Peter Runyon, Sara Fisher, and Jon Stavney. All three county coinmissioners are
committed to providing essential basic services for older adults and supporting the
development of an assisted living facility. Eagle County Aging and Adult Services is
dedicated to maintaining quality programs for older adults and continuing efforts to
expand care options to meet the needs of adults in our cominunity.

Services. The participating entities include Eagl~County Aging and Adult
Services and the El Pomar Foundation. The grant appliea~i~ii~ill-~n°rt[ 6 older
adults for the Home Connections Program and provide volunteer mileage
reimbursement for the Volunteer Services Medical Transportation Program.

7) Purpose: The grant request to the El Pomar Foundation is for $10,480.00. This
financial assistance will help to expand two vital programs that support safe
independent living for older adults in Eagle County. Aging and Adult Services is
requesting $6480.00 for the Hoine Connections Program which will provide
bimonthly free housekeeping services to 6 older adults who qualify. If Eagle
County Aging and Adult Services is awarded this grant, Eagle County will be able
to serve a total of 10 clients with Home Connections services in 2010. Aging and
Adult Services is requesting $4000.00 for the Volunteer Services Medical
Transportation Program. This amount will allow Volunteer Services to reimburse
volunteers who provide transportation far older adults to essential inedical
appointinents for 10,000 miles. If Aging and Adult Services receives this reward,
Eagle County will be able to reimburse volunteers for a total of 20,000 miles in
2010.
8) Schedule: There is no due date for the El Pomar Foundation grant application.
Agin~ and Adult Services plans to submit the grant by Septeinber 15.
9) Financial considerations and New World Line Item: The grant request is for
$10,480.00 towards cun~ent services. The agreement does comply with Eagle
County's Purchasing / Bidding / Request for Proposal Guidelines set forth in
Resolution 2006-071.
