Eagle County Aging and Adult Services is requesting United Way of Eagle River Valley Program Funding for Home Connections, a small program designed to support local older adults to live safely and independently in their own homes by providing free, bimonthly housekeeping assistance. It is the mission of Eagle County Aging and Adult Services to provide leadership to create-and sustain an aging well community that promotes social and civic engagement, planning along the lifespan, optimizes physical and mental independence, supports community caregivers and addresses the basic needs and safety concerns of older adults and persons with disabilities. Currently, Eagle County Government in collaboration with the Alpine Area on Aging is the main source of services for older adults in the county. Eagle County Aging and Adult Services includes programs for discounted nutritious meals as well as health awareness and disease prevention activities two times a week at meals sites in Basalt, Eagle and Minturn. Eagle County Government also offers options for independent senior housing, caregiver support groups, vision support groups, and social/ educational activities. To address both adult protection requirements and the need for resource coordination, the Aging and Adult Services case manager investigates abuse, neglect, and exploitation of seniors and disabled adults in the community as well as coordinates carelcase management services to meet the needs of at-risk adults who strive to remain independent and safe in their homes. Eagle County Volunteer Services rounds out the Aging and Adult Services Department by supporting older adults with options for volunteering in the community through the Rural Resort RSVP Program. Volunteer Services also recruits and organizes a network of volunteers of all ages to help with many aspects of Aging and Adult Services. The Home Connections program expands the capacity of Aging and Adult Services to address a growing gap in resources for older adults in Eagle County. It is estimated that a minimum of fifty adults aged sixty years and older in Eagle County are being underserved and are in at-risk living situations due to the absence of community; non-medical, in-home services, including housekeeping and home maintenance assistance and non-skilled nursing for bathing and meal preparation. At least ten of those individuals are at imminent risk of institutionalization. Eagle County Home Connections meets this community need by providing free housekeeping two times a month to local residents who meet eligibility requirements for age and income and who demonstrate physical or mental incapacities that require intervention to remain safely in their home. It is a goal of Home Connections to prevent accidents and injuries of the older adults, as well as prevent the involvement of more costly adult protection services and the entry into an assisted living facility. Describe the program to be funded. If this is a collaborative, interagency project, please describe each agency's role in the program. Please describe how the need for the program was determined. Home Connections will grant free housekeeping services to older adults living in Eagle County who live at or below 250% of poverty and demonstrate difficulty maintaining activities of daily living due to either physical or mental health incapacities. Currently, the Aging and Adult Services case manager takes referrals from the~community on any older adult who is need of resources and support to live more safely in their home. The case/ care manager meets with each client who is interested in and maybe eligible for Home Connections and completes both the Activities of Daily Living and Individual Activities of Daily Living Assessment scales. In order to be eligible to receive Home Connections services, the older adults need. to have difficulty completing at least 50% of the tasks on the assessment scales and also meet income eligibility requirements. Once assessments are finalized, the case manager and the Home Connections coordinator meet to determine if the adult is eligible and of highest need for service. The Aging and Adult Services case manager completes a home visit with each older adults enrolled in Home Connections to work in collaboration to complete a service agreement form stating that the older adults wants to participate in the Home Connections services, giving permission for the housekeeping service in the home and designating which housekeeping duties the older adult would like assistance with. The cleaning services will be provided in a two hour period twice a month and will include many of the following duties: vacuuming, cleaning kitchen and bathroom floors, cleaning bathroom sinks and bathtubs, changing linens and any other related assistance the older adult may need. Home Connections is an effort to create a continuum of care and service options for older adults living independently in Eagle County. There are limited programs in the area offering non- medical, in-home assistance for older adults. Our local home health agency supplies skilled nursing services only and there are no non-skilled nursing, adult day care and respite care agencies in Eagle County. Also, there is no assisted living facility in Eagle County. Often community residents aged 60 and older are forced to move out of their communities to receive a higher level of care or they stay at home putting themselves at risk of accident, injury or illness. It is estimated that there are 4,319 residents over the age of 60 living in Eagle County. Approximately, 757 of these residents live at 185% of the poverty rate or lower. It is difficult for many local seniors to afford housekeeping assistance on their own. The lack of in-home services for older adults places an extensive burden on caregivers, family members and friends, who attempt to care for these older adults in the absence of financial and programmatic resources. In addition to existing Aging and Adult Services programs for nutrition, transportation to essential medical appointments and errands, and volunteer services, Home Connections forms a stronger safety net of support to older adults. 2. How many Eagle County residents will you serve in this program? What are the demographics of these clients? Provide historical data if available, and projections for the future. Please use an unduplicated headcount -- that is, count each individual or family only once for each year. Home Connections will give housekeeping services to ten adults aged 60 years and over who meet eligibility requirements. The older adults will need to be living independently however also demonstrate mental and physical limitations that make it difficult for them to live safely on their own. In addition, the residents who receive Home Connections services must be living at 250% of poverty or lower. Special consideration will also be given to residents who are without family or caregiver support. During the last year, Home Connections provided housekeeping help to seven different older adults. These residents range in age from b9 to 87 years and live throughout the county. Five of the individuals are males and two are females. Five of the individuals use assistance for walking including a cane, walker and/or wheelchair. All of them live independently and have limited family support. Here are a couple of examples of the older adults and their current situations. Older adult #1 is a female, aged 79 years, living in Eagle and using a walker and wheelchair due to stenosis of the spine and lupus. Older adult #2 is a male, aged 69 years, living in Eagle who has difficulty walking and using the right side of his body due to a recent stroke, and older adult #3 is a male, aged 79 years, living in Eagle who has kidney failure and has to travel out of the county an average of three days a week for dialysis. For each of these individuals, the house cleaning proprietor vacuums the floors, cleans the bathtub and/or shower, toilet and sink and mops the floors, which due to mobility problems and poor health, these clients cannot manage on their own. Home Connections has met an essential need for each of these individuals. Eagle County Aging and Adult Services has marketed Home Connections on a small scale at the senior meal sites in Minturn and Eagle. At this time there are three additional residents on the waitlist to receive Home Connections services. In addition, Eagle County has not begun to expand this program in the El Jebel-Basalt area where there are an estimated five residents in need of this service. If Eagle County Aging and Adult Services could also develop Home Connections to include not only older adults but also disabled adults of any age in Eagle County, it is estimated that at a minimum there would be an additional five individuals eligible for services. Aging and Adult Services aims to support ten clients with housekeeping services in the next year and expand Home Connections to support twenty local residents within the next two years. 3. How will this program be staffed? Home Connections is a collaborative effort of already existing Aging and Adult Services staff. The Volunteer Center Coordinator administrates Home Connections including; completing program funding grants, organizing contracts for service, budgeting, communicating and arranging billing and payment with the housekeeping contractor, and marketing. The Aging and Adult Services Case Manager provides case management for older adults receiving Home . Connections services including collecting and assessing new referrals, completing initial and follow-up assessments to determine who is eligible and supporting with the coordination of starting the housekeeping in the older .adult's homes. The cleaning services are supplied by licensed and insured housekeeping contractors. The current Home Connection's contractor has experience both volunteering and working with older adults and enjoys their company and assisting them. Eagle County pays the contractor $20.OO~r for housekeeping services as well as an additional $10.00 amonth/ client for cleaning supplies. 5. What is the budget for this program? What proportion of the budget do you propose to fund with United Way contributions? If this is a collaborative project,. what is the dollar allocation to each participating agency? Eagle County Aging and Adult Services is requesting $8000.00 to fund the housekeeping services for the Home Connections program which includes cleaning two times a month for two hours a session to ten clients. Eagle County will pay for the cleaning supplies for each client for the year as well as continue to budget for the staff time for Home Connections. Please see the budget attached below for more details and information. How will the success of this program/project be quantified, measured and reported? (The United Way will request results prior to 4th quarter allocation of granted funds.) Eagle County Aging and Adult Services will report on the number of clients served using United Way Program Funds as well as include ,information on qualitative data on the older adults who receive the housekeeping assistance. The successes of the Home Connections program will be measured throughout the time that clients receive services. Program successes will be quantified by the number of clients who do not have accidents or injuries and do not require movement into a higher level of care while receiving the housekeeping services. 7. What are your alternatives for funding and/or program management if the program cannot be funded to .the requested amount? Currently, Eagle County Aging and Adult Services comprises of programming for nutritious meals, medical and errand transportation and volunteer support. However, there are older adults in the community who need are in need of critical in-home assistance including house cleaning. Eagle County Aging and Adult Services plans to budget for Home Connections in 2010 however given the current economic climate it is difficult to say how much will be allocated for this program. In addition, Eagle County Aging and Adult Services will pursue additional grant support through the El Pomar Foundation and Alpine Area on Aging by September 2009. It is important to say that without the support from United Way, Aging and Adult Services will not be able to provide Home Connections to older adults. 8. How will the United Way of Eagle River Valley be recognized as a benefactor of this program? In June 2008, Eagle County finalized a Volunteer Services brochure that includes the United Way logo. Volunteer Services shares the brochure with individuals interested in volunteering throughout Eagle County as well as multiple organizations. In addition, the brochure is available to the public in multiple Eagle County offices. Currently, Aging and Adult Services does not have a brochure on their programs however it is a goal to complete one in the next year and the United Way logo can be included on this brochure. 9. Please provide additional information that you feel will be helpful to the United Way Board of Directors. PART III OVERVIEW of LAST YEAR'S PROGRAM If UWERV funded or partially funded a program of your agency in 2008-2009, please give an overview of that program and the clients served. Explain the program's goals, and whether or not they were met. Are you seeking United Way funds to continue this program? If not, will it be continued using .other funds? Use measurable and clear facts. In May 2008, Eagle County requested $5450.00 from the United Way of Eagle River Valley to launch Home Connections for the first year and was awarded $2500.00. Eagle County originally planned to use United Way Program and Eagle County funds to provide for comprehensive services in Home Connections including volunteer supports for older adults, home maintenance assistance and housekeeping in the senior's homes, and marketing for the program. To accommodate for multiple changes in Eagle County management and economic constraints, Aging and Adult Services narrowed the focus of Home Connections to include housekeeping only, decidedly the most essential and needed assistance for older adults in Eagle County. Beginning in January of 2009, Eagle County Aging and Adult Services began providing Home Connections to three eligible older adults. Of these three initial clients, one older adult had to be placed in a higher level of care for medical attention. However, this older adult is scheduled to return home and begin receiving the housekeeping help again soon. By April 2009, Eagle County enrolled a total of 6 regular clients and all of these individuals remain independent in their own homes. Although Home Connections was slow to launch, the program is officially serving older adults in Eagle County. During the last year, contracts for the housekeeping service have been completed and client agreements for services are in place for 6 clients currently. Eagle County is excited to continue providing this essential program for older adults in our communities and to work to expand and improve the program to better meet this need in our valley. Aging and Adult Services currently has $2,000 budgeted for Home Connections. Currently, the Volunteer Center Coordinator donates 10 hours a month of staff time to administrate Home Connections. The Volunteer Center position is currently part of the 2009 budget.
