CONTRACT AMENDMENT 1
THIS AMENDMENT, made this 23rd day of October. 2007. by and between the State of Colorado for the use and
benefit of the DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT, whose address or principal
place of business is 4300 Cherry Creek Drive South, Denver, Colorado 80246, hereinafter referred to as "the
State"; and, BOARD OF COUNTY COMMISSIONERS OF EAGLE COUNTY, (A Political Subdivision of
the state of Colorado), whose address or principal place of business is P O Box 850, Eagle, Colorado 81631.
hereinafter referred to as "the Contractor".
FACTUAL RECITALS
Authority exists in the Law and Funds have been budgeted, appropriated, and otherwise made available and a
sufficient unencumbered balance thereof remains available for payment; and
Required approval, clearance, and coordination has been accomplished from and with all appropriate agencies; and
The parties entered into a contract dated 4/20/2007, for Family Planning Services. The purpose for this amendment
is described below.
The State promises to increase the amount of funds to be paid to the Contractor by Nine Hundred Nine Dollars
909.00 during the current term of the Original Task Order Contract in exchange for the promise of the Contractor
to perform the increased work under the Original Task Order Contract.
The State is adding a Funding Letter to the Original Task Order Contract.
NOW THEREFORE, it is hereby agreed that
1. Consideration for this amendment to the original contract, 08 FLA 00060, PO FLA FHS0800060, dated
4/20/2007 consists of the payments which shall be made pursuant to this amendment and the promises and
agreements herein set forth.
2. It is expressly agreed by the parties that this amendment is supplemental to the original Contract, referred
to as the `original contract," which is, by this reference incorporated herein, that all terms, conditions, and
provisions thereof, unless specifically modified herein, are to apply to this amendment as though they were
expressly rewritten, incorporated, and included herein.
3. It is agreed the original contract is and shall be modified, altered, and changed in the following respects
only:
a. The maximum amount payable by the State for the work to be performed by the Contractor during
this current term is increased by Nine Hundred Nine Dollars ($909.00) for an amended total
financial obligation of the State of FIFTY-FIVE THOUSAND THIRTY-ONE DOLLARS
($55.031.001. The revised Budget is incorporated herein by this reference and identified as

Attachment B-2. The Original Task Order Contract is modified accordingly. All other terms and
conditions of the Original Task Order Contract are reaffirmed.
b. There shall be a new paragraph #7 added to Exhibit A to read as follows:
7. The state may allocate more or less funds available on this contract using a Funding Letter
substantially equivalent to Exhibit G and bearing the approval of the State Controller or his
designee. The funding letter shall not be deemed valid until it shall have been approved by
the State Controller or his designee.
c. There shall be anew paragraph #14 added to Exhibit B to read as follows:
14. The contractor shall use the increased funding by December 31, 2007 to purchase efficacious
methods of contraception, including one or more of the following Evra, Nuvaring, Depo
Provera, Implanon, IUDs and/or oral contraceptives.
Male reproductive health services grant to be used for screening males of any age for
chlamydia and for treating any males testing positive as well as their partners.
The contractor shall keep a separate file of all invoices paid with these funds, as back up, in
the event of an audit of the utilization of this supplemental funding.
4. The effective date of this amendment is upon approval of the State Controller or November 26, 2007,
whichever is later.
5. Except for the "Special Provisions", in the event of any conflict, inconsistency, variance, or contradiction
between the provisions of this amendment and any of the provisions of the original contract, the provisions
of this amendment shall in all respects supersede, govern, and control. The "Special Provisions" shall
always be controlling over other provisions in the contract or amendments. The representations in the
Special Provisions concerning the absence of bribery or corrupt influences and personal interest of State
employees are presently reaffirmed.
6. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE CURRENT FISCAL YEAR
ARE CONTINGENT UPON FUNDS FOR THAT PURPOSE BEING APPROPRIATED, BUDGETED,
AND OTHERWISE MADE AVAILABLE.

CRS 2430-202 requires that the State Controller approve all state contracts. This limited amendment is not
valid until the State Controller, or such assistant as he may delegate, has signed it. The contractor is not
authorized to begin performance until the contract is signed and dated below. If performance begins prior to
the date below, the State of Colorado may not be obligated to pay for goods and/or services provided.
