ADDENDUM TO THE AGREEMENT BETWEEN
EAGLE COUNTY
AND
ASPEN CENTER for WOMEN'S HEALTH
THIS Addendum, signed August 8, 2006 is to the AGREEMENT made on the 23rd day
of May, 2006, by and between the County of Eagle, State of Colorado, a body corporate
and politic, "County" and, Eagle Care Medical Clinic, hereinafter "Contractor."
Article III: COMPENSATION is hereby amended to read:
A. Eagle County will reimburse the Contractor $375 per prenatal client, for up to 50
prenatal clients who will deliver in 2006. The maximum amount of payment under
this agreement is $ 18,750. The contractor will be reimbursed monthly based on the
number of prenatal clients referred for service by the County who have delivered and/or
completed their pre-natal services through the Contractor. Eagle County will pay directly
for routine medical tests, including ultrasounds, and authorized lab costs associated with
the prenatal care.
All other terms of the agreement entered into on May 23,2006 remain in full force and
effect.
IN WITNESS WHEREOF, the parties hereto have executed this Agreement on the date
first set forth above. The parties hereto have signed this Agreement in triplicate. Two
counterparts have been delivered to County and one to the Contractor.
