COLLABORATIVE AGREEMENT FOR PHYSICIAN OVERSIGHT OF
EAGLE COUNTY PUBLIC HEALTH FAMILY NURSE PRACTITIONER
This Agreement ("the Agreement"), made this 27th day of June, 2006, between Eagle County,
Colorado, Renee Dubuisson, R.N., and Kent Petrie, M.D.,
WITNESSETH:
Whereas, Eagle County ("County"), through its Department of Health and Human Services
("HHS"), works to promote the health, safety and welfare of County residents of all ages; and
Whereas, County has determined that it would be in the interest of the health, safety and welfare
of County residents for County to provide a qualified Family Nurse Practitioner ("Nurse") to
work with established clients of HHS at the Avon Public Health Clinic; and
Whereas, Renee Dubuisson, RN, FNP-C, a Certified Family Nurse Practitioner, wishes to serve
as County's Nurse based at the Avon Public Health Clinic; and
Whereas, Colorado law requires that Practitioners work under the supervision of a licensed and
qualified physician; and
Whereas, Kent Petrie, M.D. ("Physician"), a Board Certified Family Practice physician fully
licensed to practice in Eagle County, is willing to serve as the supervising physician for Nurse in
compliance with Colorado law;
Now, therefore, County, Nurse and Physician agree as follows:
1. Physician will supervise Nurse in her performance of her duties as County's Nurse as
required by Colorado state law, specifically including the Nurse Practice Act, C.R.S. 12-
38 101 et seq., and any other applicable laws and rules regulating the authority and
responsibilities of advanced practice nurses.
2. Nurse will work under and in accordance with the supervision of Physician as set forth in
paragraphs 1 and 4 of this Agreement and will not engage in any activities forbidden to or
inappropriate for her as a Nurse Practitioner in Colorado. Nurse will consult with
Physician from time to time and as needed in order to assure that her work is in
accordance with Colorado law and the directions of Physician.
3. County will cooperate with Physician and Nurse in enabling them to fulfill their
respective responsibilities under the Agreement.
4. Nurse's duties under this Agreement will be to provide acute primary health care to
established clients in the Avon Public Health Clinic as needed. The focus of the care will
be on health promotion and prevention of illness and injury for County residents.
Services provided will include education and health counseling, history and physical
assessment, diagnosis and treatment. Subject to the provisions of paragraphs 1 and 2 of
this Agreement, Nurse will function independently with the standards of practice
established by the American Nursing Association ("ANA"). Accordingly, routine written
orders and telephone consultation will be provided independently by Nurse without a
requirement for co-signature of Physician, but Nurse will collaborate and consult with the
Physician and other health care specialists as necessary and appropriate. Clinic
performance and quality of care will meet all applicable ANA standards. Pre-established
criteria such as protocols and standards will be used as a guide to provide consistent and
comprehensive care to County residents. Retrospective and concurrent chart review will
be done to ensure that quality care is given; Physician will review ten percent (10%) of all
charts, and provide feedback to Nurse based on such review. Nurse has and may exercise
prescriptive authority and prescribe medications in accordance with the rules of the State
of Colorado Medical Board.
In witness whereof, County, Nurse and Physician have executed this Agreement as of the date
above.
