Proposal: Funding Request in the amount of $13,400.00 for start up funds for an
Adult Services Caseworker Position. This represents .25 FTE.
HOW FONDS WILL BE USED FOR SENIOR SER\tlCE:
Eagle County Adult Case Management is a consumer focused collaborative program,
which assesses individual needs, and plans, implements, coordinates, monitors, and
evaluates options and services, using formal and informal sources, to meet those needs.
The Adult Services Case Worker will promote the ability of the older or vulnerable
person and his or her caregiver(s) to make informed decisions in addressing needs using
high quality and cost effective services. Per NAPIS service definition, the case worker
will offer assistance either in the form of access or care coordination in circumstances
where the older persons and/or their caregivers are experiencing diminished functioning
capacities, personal conditions or other characteristics which require the provision of
services by formal services providers. Prior to the initiation of this program there were no
available resources for adult case management services in Eagle County. This program is
of particular importance to those individuals who are socially and geographically isolated
and of the greatest economic need. According to the Colorado Department of Human
Services; Aging and Adult Services, September 2004,2% of Eagle County older adults
were at risk of institutionalization in 2005. That number is expected to increase to 121
persons by 2010.

County government will assume the fiscal responsibility for the remaining $40,200 and

network professionals.
