(100% attained) Assure that all children have an ongoing source of continuous, accessible medical care. If child does not have a continuous source of care, staff and parents work together to plan strategies to ensure that the family acquires a medical home. Strategies include: . At enrollment visit and first the 6 weeks of home visits assess medical status and medical home which offer continuous accessible health services. . Provide list of available doctors/dentists/agencies for families who do not have access to health services. Assist parents in selecting medical home. . Provide information about Medicaid, CHP+, HCP and other sources of financial assistance. . Staff will assist families in coordinating health services and document in record. EHS may provide transportation as a last resort to medical services. . Health Service Coordinator (HSC) will assist staff by periodically reviewing health records to ensure that recommended treatment and preventive services are being provided, and that plans are developed for treatment and follow-up. HSC will also meet monthly with individual staff to answer and assist with health concerns that the family or staff may have. . Seek assistance when needed from the Health Services Advisory Committee. - 1 - 2. Maintain the number of children receiving dental services. (100% attained) ~ Help parents understand importance of early, ongoing dental health. Provide families with education on dental health at least on a monthly basis during home visits. ~ Ensure that parents know how to get needed dental treatment. Work with families to provide a follow up plan. ~ Public Health provides dental screening per EPSDT guidelines for children birth to three. Provide list of dentists who will accept Medicaid or non-insured patients. ~ Tracking system established for checking on delivery of services 3. Maintain full enrollment. (100% attained) ~ Follow Eligibility. Recruitment, Selection, Enrollment and Attendance Procedure. ~ System in place to track enrollment and attendance on a monthly basis. ~ During staff vacancies, each home visitor adds 1-2 children per case load. Caseloads do not exceed 12 children and or families at any time. ~ Waiting list assures transitions into program to maintain full enrollment. 4. Increasing the number of home visitors with Early Childhood Education (ECE) or related degrees.
