ALPINE AREA AGENCY ON AGING
PURCHASE OF SERVICES CONTRACT RENEWAL/AMENDMENT
TITLE III PART B SERVICES OF THE OLDER AMERICANS ACT
TillS PURCHASE OF SERVICES CONTRACT RENEW AUAMENDMENT is effective the 15t day of January 2006,
by and between Northwest Colorado Council of Governments-Alpine Area Agency on Aging (hereinafter referred to as the
"Council") and Eagle County Senior Services (hereinafter referred to as the "Subgrantee").
WHEREAS, the Council and the Subgrantee entered into a Purchase of Services Contract effective January 1, 2004,
Contract #2004-Eagle to render and perfonn Transportation, Infonnation & Assistance, and Outreach services.
WHEREAS, the Council and the Subgrantee desire to amend the Purchase of Services Contract as follows:
I. The Northwest Colorado Council of Governments-Alpine Area Agency on Aging and Eagle County Senior
Services desire to renew the contract for an additional twelve (12) months beginning January 1,2006.
II. The Council hereby agrees to award grant monies (under the Older Americans Act) up to, but not exceeding
a total of $13,280.00 (Dollars) to Subgrantee for the period beginning January 1,2006 and ending
December 31, 2006.
Due to the Council transitioning to a State Fiscal Year award period with the State Unit on Aging that will
become effective July, 2006, the awarded grant monies for this period will be allocated and available to as
the Subgrantee as follows:
a. grant monies up to, but not exceeding 50% of the total award ($6,640.00) available to the
Subgrantee tor the period of January 1,2006 through June 30, 2006; and
b. the ren:ainder of the awarded grant monies to be available to Subgrantee after July 1,2006.
This award of grant monies is contingent upon full funding from the federal and state governments, as per Item
11. 0 in the original Purchases of Service Contract.
III. The Subgrantee agrees to provide, at a minimum,
14,000 units of Transportation service to an aggregate count of 500 persons
1,200 units of Infonnation & Assistance service to an aggregate count of 200 persons
300 units of Outreach service to an aggregate count of 300 persons
during the agreement renewal period.
IV. The parties hereby agree that the contribution from the Subgrantee shall consist oflocal In-Kind and Cash
Match contributions and shall be valued in the sum of$ zero (Dollars) for the agreement renewal period.
V. Subgrantee's reports will correctly indicate for Program Income when earned and expenditures of Program
Income when incurred as per Section 1O.320.10fthe Colorado Department of Human Services, Staff Manual
Volume 10 "Services for the Aging" 12 CCR 2510-1, effective April 1 , 2005 (hereinafter referred to as
Volume 10).
VI. The Subgrantee agrees to abide by all conditions, service definitions, service standards, assurance, reporting or
other requirements outlined in Volume X, and as proposed in the One-Year Renewal Application that is
attached hereto and expressly made a part hereof as Exhibit #1. Failure to comply shall be grounds for
tennination of the contract with the Subgrantee.
NOW THEREFORE, Subgrantee agrees to make every effort to serve those older individuals in greatest social and
economic need, with preference given to low-income minority individuals and older residents of rural areas and to maintain
participation of said individuals at least at the projected 2006 participation level. Subgrantee further agrees that budgets,
service delivery, service standards, and all other aspects of the services will be performed as outlined in its One-Year
Renewal Application to Council, submitted November, 2005.
That document is expressly made a part of this Renewal! Amendment. Any significant changes to the delivery of services or
budget as outlined in the One-Year Renewal Application shall be submitted to Council in writing for approval.
Subgrantee also agrees to incorporate any future changes in policy as published by the Council and/or the State Unit on
Aging into their procedures as they relate to the services provided under this contract renewal.
NOW THEREFORE, this Renewal/Amendment shall be attached to and incorporated into the terms and administrative
requirements of the original Purchase of Services Contract, dated January I, 2004. Except as amended by this
Renewal! Amendment, the terms and administrative requirements mutually agreed to in the original Purchases of Services
Contract shall remain in full force and effect.
WHEREOF, the parties hereto have entered and executed this Renewal/Amendment to be effective as of the 15t day of
January 2006.

Part 3. Service Delivery
Item I: Meeting the Needs of low-income, minority individuals and older individuals
residing in geographically isolated rural areas.
Eagle County Health & Human Services will meet these needs by:
>- conducting outreach in all areas of Eagle County
>- partnering with towns and isolated communities to identify low-income,
minority and older individuals
>- providing transportation to meal sites and health & wellness activities by:
0 using Eagle County Transportation Vans
0 reimbursing volunteer transportation providers
>- operating nutrition programs in three locations within the county to reduce
travel time and to meet the needs of low income, minority individuals and
older individuals closer to their homes
>- placing a priority for service on those who are low-income, minority, or older
individuals
>- providing outreach and services in the primary language of the participant.
Part 4. .c<\SSURANCES

Each area on aging shall provide assurances that the area agency on aging will include in each
agreement made with a provider of any service under Title III, requirements that such provider as
a subgrantee will:

A. All individuals age 60 and older shall be eligible for services. ~

B. All targeting factors shall be considered to prioritize access to services in compliance with EJ
Title III rules.
C. Meet specific objectives established by the area agency on aging, for providing services to EJ
low-income minority individuals and older individuals residing in the geographically isolated areas
within the Region 12 planning and service area.
D. To the maximum extent feasible, preference and priority in the delivery of services shall be EJ
given to older persons with the greatest social or economic need with particular attention to low-
income minority individuals and individuals who are frail, homebound by reason of illness or
incapacitating disability or otherwise isolated as defined below:
-The need caused by non-economic factors which include physical and mental disabilities,
language barriers, and cultural, social, or geographical (rural) isolation including that caused
by racial or ethnic status which restricts an individual's ability to perform normal daily tasks
or which threatens such individual's capacity to live independently.
-The need resulting from an income level at or below the Federal poverty levels.
E. All service providers shall maintain the confidentiality of protected health information, EJ
including the consumer's individually identifiable health information, and ensure that no
information about an eligible consumer is disclosed by the service provider without the informed
written consent of the consumer.

E. Where feasible and appropriate, make arrangements for the availability of services to older fXl

State Unit on Aging. This procedure will be in accordance with the policy of area agency on
aging as published in the ASU Memorandum 04-25.

advised of their right to complain about services or the denial of services, to appeal
decisions made about tr,e complaint, and that those complaints and appeals, are processed
and tracked according to the policy of area agency on aging as published in ASU

collected while ensuring the anonymity of the client and are handled and safeguarded

Coloradans Act programs as required by Volume 10 and as published in ASU Memorandum
04-29.
F. Will use the standardized Consumer Information and Assessment Form for intake U
eligibility determination and conducting assessments for consumers that receive registered
services through either the Older Americans Act or State Funded Programs for Services as
published in ASU Memorandum 04-32.
G. Will assist the area agency on aging with the State Unit on Aging's scheduled reviews U
and comparisons of service data reported to identify outliers or inconsistencies, to determine
possible causes, and where required to develop a plan to correct the issues/causes as
published in ASU Memorandum 04-33.
H. Ensure that criminal background checks are conducted for all employees, volunteers, U
and contractors who have direct contact with elderly clients as published in ASU
Memorandum 05-15.
I. As published in ASU Memorandum 05-16 provide expense detail and include supporting CJ
documentation that validates services were actually provided with provider's reimbursement
requests to area agency on aging. Provider will not bill/submit for reimbursement or count
for service delivery "no-show consumers of services" In situations where the service
provided does not provide for units of service, deliverables shall be specified in detail and
included with reimbursement requests.
