NOTIFICATION OF GRANT AWARD
Under STATE FY2006 STATE PROGRAMS FOR SENIORS
Northwest Colorado Council of Governments-Alpine Area Agency on Aging
NAME OF SUBGRANTEE: Eagle County Senior Services
CONTRACT 01-14-82-850 TYPE OF GRANT AWARD: New
THIS GRANT AWARD (hereinafter referred to in this document as "Subgrant"), entered into and
effective this July 01, 2005, by and between the NORTHWEST COLORADO COUNCIL OF
GOVERNMENTS, ALPINE AREA AGENCY ON AGING (hereinafter referred to in this document as
the "Council") and EAGLE COUNTY SENIOR SERVICES (hereinafter referred to in this document as
the "Subgrantee"), WITNESSETH THAT
WHEREAS, the ALPINE AREA AGENCY ON AGING, a division of the Northwest
Colorado Council of Governments, is mandated under the provisibns of the Older Americans
Act to develop a comprehensive coordinated service and program\delivery system to help
older persons maintain themselves in their own homes with maximum independence, dignity
and well-being; and
WHEREAS, in order to provide services under Region XII's four-year area plan, the Council
desires to engage the Subgrantee to provide certain services pursuant to the Older Americans
Act/Older Coloradans Act requirements hereinafter described in connection with the
undertaking which is expected to be financed through the State of Colorado through State
Funded Programs for Seniors.
WHEREAS, the Subgrantee desires to perform said service(s) pursuant to said Subgrant.
NOW, THEREFORE, the parties hereto understand and mutually agree that the following provisions are
part of its official Notice of Grand A ward and as such become binding upon the conduct of the service
program:
1.0 AWARD AMOUNT
The Council hereby agrees to engage the Subgrantee and the Subgrantee hereby agrees to perform
the services hereinafter set forth in connection with the program of the Council under State
Funded Programs for Seniors Funds Notification of Grant Award in the amount of Eight
Thousand Seventy-four Dollars ($8,074.00).
2.0 COMPLIANCE
The Subgrantee agrees that the program service(s) will be carried out in accordance with the
administrative, programmatic, performance, and financial procedures and requirements in the Older
Americans Act of2000 and Older Coloradans Program, the policies and procedures established for
subgrants by the Colorado Department of Human Services (Volume X-Services for the Aging 12
CCR 2510-1, effective date 4/1/2005), the Colorado State Controller policies related to
contracting, and the terms and conditions of the program grant award terms and guidelines for the

any part thereof to the Council, then this Sub grant shall be void and shall not be binding on any
parties to the Subgrant.
Unearned payments under this Subgrant may be suspended or terminated in the event that the
Subgrantee refuses to accept additional terms or conditions to this Subgrant that may be imposed
by CDHS, HHS, or State after the effective date of the Subgrant.
9.0 REPORTING REQillREMENTS
Subgrantee agreed to make reports on the forms required by the Council and in accordance with
guidelines issued by the State of Colorado and the Administration on Aging, specifically:
9.1 Performance Reports. The Sub grantee shall submit to the Council a monthly program
report, containing information in a form prescribed by the Council, throughout the term of this
Subgrant and shall be in such complete form so that all pertinent information required is
contained herein. The performance report for State Program for Seniors Fund services shall
be separate from the Title III Program Report. The Council may withhold payments to
reimburse Subgrant expenditures should the Subgrantee fail to meet said requirements.
9.2 Financial Reports. The Subgrantee shall submit to the Council a monthly financial report,
containing information in a form prescribed by the Council, throughout the term of this
Subgrant and shall be in such complete form so that all pertinent information required is
contained there. State Program for Seniors Fund expenditures shall be reported separately
from Title III fund expenditures. The Council may withhold payments to reimburse Subgrant
expenditures should the Sub grantee fail to meet said requirements.
9.3 Pro2ram Income. Program Income earned by the Subgrantee within a specified reporting
period must be reported on the prescribed financial reporting form for that period.
10.0 FISCAL CONTROLS, RECORDS, AUDITS AND INSPECTIONS
10.1 Internal Controls. Subgrantees shall ensure that fiscal controls are established and
maintained for itself. Subgrantees shall ensure that minimum standards and procedures are
established for the responsible collection of, handling and safeguarding of participant
contributions, and non-eligible recipient fees.
10.2 Records. The Subgrantee agrees to maintain and retain all accounts and documents deemed
responsible, until formal written notice has been received.
10.3 Accountin2 Records. Records, which identifY adequately the source and application of
funds for Subgrant activities, will serve to permit determination at any time on the status of
funds within the award, including the disposition of all monies received, and the nature and
amount of all charges claimed to be against such funds.
10.4 Audits and Inspections. During the Subgrant award period, the retention period, and as
long thereafter as the records are maintained, at any time during normal business hours, the
Council, CDHS, HHS, the Comptroller General of the United States or any of their
authorized representatives and the State shall have the right of access to any books,
documents, papers or other records of the Subgrantee with respect to all matters covered by
the Subgrant and the Older Americans Act/Older Coloradans Act in order to make audit,
examination, excerpts, and transcripts. If costs are disallowed, the proportion of State
Programs for Seniors funds must be returned to the Council and/or State Unit on Aging.
Representatives of the Council shall conduct on-site program and fiscal assessments of the
Subgrantee.

14.2 Mutual Termination. The parties may terminate the Subgrant upon mutual consent setting
forth in wTiting the effective date and procedures.
14.3 Termination Bv Suberantee. The Subgrantee, by written notification setting forth the
reasons and specifYing the effective date, at least 30 days prior to the effective date, may
terminate this Subgrant. Reasons for termination may include circumstances beyond the
control of the Subgrantee. If the Subgrantee terminates a portion of the Subgrant, the
Council, at its option, and if it determines the remaining portion would not accomplish the
purposes of the Subgrant, may terminate the Subgrant in its entirety.
15.0 PERSONNEL
15.! Personnel. The Subgrantee assures that it has, or will secure at its own expense, all
personnel required in performing the services under this Subgrant. Such personnel shall not
be employees of the Council.
15.2 Qualifications. All of the services required herein under will be performed by the
Subgrantee or under its supervision, and all personnel engaged in work shall be fully
qualified and shall be authorized under State and local law to perform such services.
15.3 Subcontract. None of the work or services covered by this Subgrant shall be subcontracted
without the prior written approval of the Council.
15.4 Equal Employment Opportunity/Handicapped. The Subgrantee agrees to comply with
Executive Order Number 11426 entitled "Equal Employment Opportunity" as supplemented
in the Department of Labor Regulations (41 C.F.R. Part 84) and to execute such provisions
as are required. The Subgrantee will not discriminate in employment in the basis on
handicap against any qualified handicapped person and agrees to take positive steps to
employ and advance in employment qualified handicapped persons and to comply with the
Department of Health and Human Service Regulations (45 C.F.R. Part 84).
16.0 OTHER TERMS AND CONDITIONS
16.! Unearned Monies. The Subgrantee understand that, if there is Federal/State cash unearned
at the time the program is terminated, this amount must be returned to the Council, unless
the Council otherwise stipulates. Conditions for earning Federal/State monies include:
(1) Expenditures of funds; and
(2) Matching Federal/State monies at the negotiated rate.
(3) Contributions (Program Income)
16.2 One-Time Fundine. The Subgrantee understands that this Subgrant is made for the period
and amounts stated in this Subgrant. This Subgrant in no way implies further funding, which
is contingent upon the availability of funds and approval of future program applications.
16.3 Equipment. Equipment purchased all or in part with funds under the Older Americans Act
and State General Funds is subject to the transfer and equipment management regulations set
forth respectively in Section 74:136; 74:139 and 74:140 of the Code of Federal Regulations,
Title 45, Part 74 (45 CFR 74).
17.0 INSURANCE
Subgrantee shall obtain and maintain throughout the term of the Subgrant, at Subgrantee's sole /'
expense, workers compensation and/or employer's liability insurance in an approved company or
companies, to cover all classifications of work and workers herein contemplated in accordance
with applicable law and regulation. Subgrantee will also carry and maintain throughout the term

Eagle County Senior Services is applying for State Funding in the amount of$8,124 for the funding
cycle 2005/2006.

The Golden Eagle Senior Center will undergo major renovation and expansion in the summer and

,. allow greater accessibility, encourage broader program development and improve
operational efficiency; and
>> provide adequate space for future demographic changes in Eagle County's older adult
population.
State Funded dollars will assist in replacing older equipment with new, professional grade appliances
- an institutional size range/oven, a new dishwasher and a new ice dispenser. Eagle County's costs
in the renovation and expansion of the facility will provide a cash march.'

New appliances will save cook and volunteer staff time, be more energy efficient, and dernonsrrate
to participants that they are participating in a quality nutrition program. With the growing demand
for congregate meals at the Golden Eagle Senior Center, the higher quality, more cost efficient
appliances would require only moderate increases in overhead costs and staff operational time as the
program serves larger ntunbers of meals.

Information, Assistance & Referral (IA&R) Outreach Programs:
For the 2005/2006 grant funding period;. Eagle County Senior Services equests $1,124 for IA&R
Outreach Programs. IA&R Outreach Programs are conducted at three sites in Eagle County. and
funds would be divided equally between the sites..
Eagle County Senior Services has successfully reached seniors and family members in Basalt, Eagle
and Avon this year. Outreach efforts in 2004/2005 included a 50's style parry, a mOlUltain polka
celebration. and a pancake breakfast that even drew members ofllie U.S. Secret Service! A central
focus of outreach efforts is dissemination of information on federal, state and local programs serving
o~der adults. In total, Eagle County Senior Services was able to communicate our message to well
over 1,000 people in our communities; with hundreds ofIA&R units of service directly attributable
to these outreach efforts.
With the confidence gained from our collective experience during the 2004/2005 grant funding
cycle, we will pursue lA&R outreach programs that provide seniors and family members with
informati.on to assist them in the areas of aging. caregiving, financial, legal, nutrition, transportation
and healthJwe1lness issues.
This year Eagle COWlty Senior Services will undertake three to four IA&R Outreach events. HHS
has requested an additional site coordinator who is bi-lingual (Spamsh/English) and culturally
competent. If this staff position is approved by the County, Eagle County Senior Services
anticipates expansion ofthe outreach program with local Hispanic seniors and their extended family.

Certifications and Assurances:
The Alpine Area Agency on Aging (AAAA) shall assure that services authorized under the Older
Americans Act are delivered through a network of local service providers under grant or contract with the
AAAA.
Failure to comply with the requirements of these rules by any service provider inCluding sub~grantee5,
shall be grounds for termination of the subgrant. As a condition for receipt of funds, all service providers,
inclUding sub-grantees, shall:

Low Income/Minority individuals will be targeted in compliance with litle III rules. ~
All subgranted state funds will be fully expended within the State Fiscal Year ending
June 30th, 2006.
All subgranted state funds will be used as overmatch funds and will increase service ~
units and clients served to the over all senior services and programs.
All services provided shall meet the service standards described in Volume X (12 tP
CCR 2510-1 effective 4/1/2005) at Sections 10.412 -10.419, ~
Service providers receiving State Funded Programs for Seniors funds to render
services under the program shall meet the service provider requirements described in
Volume X (12 CCR 2510-1 effective 4/1/2005) at Sections 10.260.2-10.260.6.
Provide the Alpine Area Agency on Aging, within required timeframes. statistical and ~
other information which the AAA requires to meet its planning, coordination,
evaluation, and reporting requirements established by the SUA. ez(
All individuals age 60 and older shall be eligible for services.
Provide clients with an opportunity to contribute to the cost of the service in a manner ~
that allows the client to contribute voluntarily and anonymously, and that no eligible
client shall be denied service because of a decision not to contribute. ~
Wjth the consent of the older person. or his or her legal representative, bring to the
attention of appropriate officials for follow-up, conditions or circumstances which place
the older person, or the household of the older person, in imminent danger.
Where feasible and appropriate, make arrangements for the availability of services to l)(
older persons in weather related emergencies or ather emergency situations.
Assist clients in taking advantage of benefits under other programs. ~
Establish complaint and appealS procedures and inform clients of their righHo ~
complain and receive a written response at the provider level.
Inform clients/customers of their grievance rights and their right to request and receive ti'

Establish a waiting list for services. ~
