HomeMy WebLinkAboutC05-038 NWCCOG to6 - 00 fy" ;;):~ NORTHWEST COLORADO COUNCIL OF GOVERNMENTS ALPINE AREA AGENCY ON AGING TITLE III OF THE OLDER AMERICANS ACT PURCHASE OF SERVICES CONTRACT RENEW AL/AMENDlVIENT THIS PURCHASE OF SERVICES CONTRACT AMENDMENT is effective the 151 day of January 2005, by and between Northwest Colorado Council of Governments-Alpine Area Agency on Aging (hereinafter referred to as the "Council") and Eagle County Senior Services (hereinafter referred to as the "Contractor"). WHEREAS, the Council and the Contractor entered into a Purchase of Services Contract effective January 1,2004, Contract #2004-Eagle to render and perform Assisted Transportation, Transportation and Information & Assistance services. WHEREAS, the Council and the Contractor desire to amend the Agreement as follows: I. The Northwest Colorado Council of Governments-Alpine Area Agency on Aging and Eagle County Senior Services desire to renew the Agreement for an additional twelve (12) months beginning January 1,2005. II. The Council hereby agrees to award additional grant monies (under the Older Americans Act and/or State General Funds) up to, but not exceeding $13,280.00 (Dollars) to Contractor for the period beginning Januarv I. 2005 and ending December 31. 2005 rSee Attachment]. This award is contingent upon full funding from the federal and state governments, as per Item 11.0 in the original Agreement. Ill. The Contractor agrees to provide, at a minimum, during the agreement renewal period [See Attachment]: 12,000 units of direct Assisted Transportation service to 23 unduplicated individuals; 14,000 units of direct Transportation service to 1,550 total aggregate persons; 180 units of direct Information & Assistance to 560 total aggregate persons; IV. The parties hereby agree that the contribution from the Contractor shall consist oflocal In-Kind and Cash Match contributions and shall be valued in the sum of $2,770.00 (Dollars) for the agreement renewal period. Contractor's reports will correctly indicate for Program Income when earned and expenditures of Program Income when incurred. WHEREAS, the Performance Audit of the State Services for Older Coloradans Department of Human Services, completed in June 2004, requires compliance by the Council and the Contractor with the following policies and procedures established for grants by the State: V. Eagle County Senior Services will present Council with proof of their procedure to ensure that a waiting list for the services offered under this contract is developed, maintained and made readily available for review by Council and/or the State Unit on Aging. This procedure will be in accordance with the policy of Council as published in the ASU Memorandum 04-25. VI. Eagle County Senior Services will present Council with proof of their procedure to ensure that applicants to, or consumers of, their services are advised of their right to complain about services or the denial of services, to appeal decisions made about the complaint, and that those complaints and appeals, are processed and tracked according to the policy of Council as published in ASU Memorandum 04-27. VII. Eagle County Senior Services will present Council with proof of their procedure to ensure that client voluntary contributions received are responsibly collected while ensuring the anonymity of the client, and C:\Subgrantees (TitlelII.B)\2004 Purchase Contract Renewal.doc I are handled and safeguarded according to the policy of Council as published in ASU Memorandum 04- 28. VIII. Eagle County Senior Services will present Council with proof of their procedure to ensure that non- eligible recipient fees are diligently collected, handled and safeguarded according to the policy of Council as published in ASU Memorandum 04-28. IX. Eagle County Senior Services acknowledges that it is fully aware of the requirement of a periodic on-site assessment by the Council and/or the State Unit on Aging of Older American Act and Older Coloradans Act programs as required by Volume 10 and as published in ASU Memorandum 04-29. X. Eagle County Senior Services will use the standardized Consumer Information and Assessment Form for intake eligibility determination and conducting assessments for consumers that receive registered services through either the Older Americans Act or State Funding for Senior Services as published in ASU Memorandum 04-32. XL Eagle County Senior Services agrees to incorporate any future changes in policy as published by the Council and the State Unit on Aging into their procedures as they relate to the services offered under this contract. NOW THEREFORE, this Amendment shall be attached to and incorporated into the terms and administrative requirements of the original Purchase of Services Contract, dated January 1,2004. Except as amended by this Amendment, the terms and administrative requirements mutually agreed to in the original contract shall remain in full force and effect. WHEREOF, the parties hereto have entered and executed this Amendment to be effective as of the 15t day of January 2005.
