ADDENDUM to the AGREEMENT BETWEEN
THE COUNTY OF EAGLE, STATE OF COLORADO
AND
VALLEY PARTNERSHIP for DRUG PREVENTION
THIS Addendum, signed June 1, 2004 is to the AGREEMENT made on the 9th day of
September, 2003, by and between the County of Eagle, State of Colorado, a body corporate and
politic, "County" and, the Colorado West Regional Mental Health Center, hereinafter
"Contractor."
1. AGREEMENT is hereby amended to read:
This Agreement shall commence on September 1, 2003, and shall end on June 30, 2004. The
maximum amount of this contract for professional technical assistance services shall not exceed
$ 14,309. Expenses for program supplies and travel expenses for local travel and participation in
meetings with state partners are included in this contract.
All other terms of the agreement entered into on September 9, 2003 remain in full force and
effect.
IN WITNESS WHEREOF, the parties hereto have executed this Agreement on the date first set
forth above. The parties hereto have signed this Agreement in triplicate. Two counterparts have
been delivered to County and one to the Contractor. .
