This Contract Option to Renew Letter is issued pursuant to paragraph C9. of the Contract identified by contract routing
number 03 FAA 00363 and oon>zact encumbrance mmnber PO FAA WIC0300363. This Contract Option to Renew Letter is
between the COLORADO DEPARTMENT OF PIIBLIC HEALTH AND ENVIItONMENT and the BOARD OF
COIINTY COMMISSIONERS OF EAGLE COIINTY. The Contract has been amended by Contract Change Order Letter
1 contract rooting number 03 FAA 00808, and Coilract Change Omer Letter ?~ contract muting number 04 FAA 00205.
The Contract, as amended, is referred to as the "Original Coilracl". This Contract Option to Renew Letter is for the
renewal term of October 1.2003, through September 30.2004. The mairimum amount payable by the State for the work
to be performed by the Contractor daring this renewal tram is ONE HIINDRED SIX THOII5AND NINE HIINDRED
NINETY THREE DOLLARS ($106993.00) for an amended total financial obligation of the State of TWO HONORED
SIXTEEN THOIISAND NINE HIINDRED FIFTY ONE DOLLARS ($216951.00), The first sentence in paragraph
C_l. of the Original Contract is modified a~xordingly. All other terms and conditions of the Original Contract are reaffirmed.
This Contract Option to Renew Letter is effective upon approval by the State Controller, or designee, or on October 1.2003.

contract routing number 03 FAA 00808. This Contract Change Order Letter is between the COLORADO
DEPARTMENT OF Pl7BLIC HEALTH AND ENVIRONMENT and the BOARD OF COIINTY CONIlVIIS5IONER5
OF EAGLE COiTNTY. This Contract Change Order Letter is for the current term of October. i. 2002, through
September 30.2003. The maximum amount payable by the Slate for the work to be performed by the Contractor during this
current term is increased by TWO THOII5AND NINE HUNDRED SIXTY FIVE DOLLARS, $2 65.00 for an
amended total financial obligati of the State of ONE HUNDRED NINE THOII5AND NINE HUNDRED FIFTY
EIGHT DOLLARS. ($109.958.00). The first sentence in paragraph C. L of the Original Co~ract is modified aarordingly.
