This Contract Change Order Letter is issued pursuant to paragraph C3. of the Contract identified as contract routing
number Ol FAA 00759 and contract encumbrance number PO FAA PRN0100759. This Contract Change Order
Letter is between the COT DORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and
BOARD OF COUNTY lOMMISSIONERS OF EAGLE COUNTY, (a political subdivision of theMate of
Colorado . The Contrac~ has been amended by Contract Option to Renew Letter #1 contract routing o~umber 03
FAA 00560 and Contract Option to Renew Letter #2 contract routing number 03 FAA 00430: The Contract, as
amended, if applicable, is referred to as the "Original Contract". This Contract Change Order Letter is for the
current term of October 1, 2002 through September 30, 2003. The maximum amount payable by the State for the
work to be performed by the Contractor during this current term is decreased by Eleven Thousand Dollars,
~$11,000.00~ for an amended total financial obligation of the State of Three Hundred Twenty Six Thousand
Five Hundred Dollars, ($326,500.00). The fast sentence in paragraph A of the Original Contract is modified
accordingly. All other terms and conditions of the Original Contract are reaffirmed. This change to the contract
shall be effective upon approval by the State controller, or designee, or on June 1.2003, whichever is later.
Please sign, date, and return a114 originals of this Task Order Change Order Letter by May 30.2003, to the attention
of: Laura Montoya. Women's Health Section, Colorado Department of Public Health and Environment, 4300
Cherry Creek Drive South, Mail Code HPDP-WHS-A5, Denver, Colorado 80246. One original of this Contract
Change Order Letter will be returned to you when fully approved.

CRS 24-30-202 requires that the State Controller approve all state contracts. This contract is not valid until
the State Controller, or such assistant as he may delegate, has signed it. The contractor is not authorized to
begin performance until the contract is signed and dated below. If performance begins prior to the date
below, the State of Colorado may not be obligated to pay for the goods and/or services provided.

This change order reduces by $11,000 the funds available for purchase ofpre-natal health
care services for non-legal residents. 001-2210-0661-000-1540 revenue
001-2210-3510-2270-2400 expenditure
PROJECT/CONTRACT DESCRIPTION: These funds are used to reimburse health care
providers for prenatal services to low income women who are not eligible for regular
prenatal Medicaid coverage due to their citizenship status.
STAFF RECOMMENDATION.: To sign the change order.
BASIS FOR RECOMMENDATION:
PROPOSED MOTION: To sign the change order with the Colorado Department of Public
Health & Environment. Contract routing number 03 FAA 00894.
