"Attar-hment *," The first sentence in paragraph D . of the Original Contract is modified accordingly. All other terms and
conditions of the Original Contract are reaffirmed. The proposed effective date of this Contract Renewal Letter is July 1, 2002
However, in accordance with section 24 -30 -202, C.R.S., as amended, in no event shall this Contract Renewal Letter be deemed
valid until it shall have been approved by the State Controller or such assistant as he may designate

PROJECT /CONTRACT DESCRIPTION
This annual contract with the Department of Public Health and Environment provides
partial funding for public health nursing services. The amount of the grant for the coming
state fiscal year is $ 58,471.48. The amount budgeted for the current year is $ 56,892.
Annual allocation of funding is based on state formulas that take into account population,
poverty rates and child population. This agreement incorporates the Governor's four
percent budget reduction.
STAFF RECOMMENDATION To enter into the contract.
BASIS FOR RECOMMENDATION This contract assists in funding basic public health
services for Eagle County.
PROPOSED MOTION To enter into the contract with the Colorado Department of Health
and Environment.
