IV,V CHANGE ORDER NO. TWO TO Concrete Works of Colorado CONTRACTOR: YOU ARE HEREBY ORDERED TO MAKE THE FOLLOWING CHANGE IN THE PLANS AND /OR SPECIFICATIONS FOR THE ABOVE DESIGNATED PROJECT. 1. DESCRIPTION OF CHANGE TO BE MADE: a) Schedules I, 11, III, IV, V - Adjust all estimated quantities to final contract quantities b) Schedule I (federal) - Rock Excavation for installation of Primary power electrical ducts c) Schedule V (non Federal) - Repair Glycol collection trench drain 2. REASON FOR ORDERING CHANGE: a) This change order will adjust estimated quantities to match final pay quantities b) The rock excavation was encountered during the installation of the primary power conduits that serve the apron flood lighting. This change also required Imported fill materail for the backfill of the electrical trench. c) The glycol collection trench drain outlet pipe was modified (lowered to a buried depth of approximately 5 feet) to prevent the freezing of the runoff fluids. 3. SETTLEMENT FOR THE COST OF THE MENTIONED CHANGE IS TO BE MADE AS FOLLOWS: Final Quantity Adjustments Schedule I (Federal) $32,716.71 (includes $ 6,462.56 for rock excavation, item (b) above) Schedule IV (Federal) ($ 740.00) Schedule V (Non- Federal) 619,618.70 (includes $8,500.00 for trench drain repair, item (c) above) TOTAL $51,605.41 THIS CHANGE ORDER INCREASES DECREASES THE CONTRACT TIME BY 0 WORKING DAYS. 4.
