Description: 1. Delete Prime Coat requirement for approach road construction
(delete contract pay item 411(1) - Liquid Asphaltic Material)
2. Increase quantity of Hot Bituminous Pavement from 307 tons to 347 tons
Purpose of Change Order:
To delete requirement for prime coat since it was impractical to cure due to low
ambient temperature and to increase pay quantity of pavement due tt'l 7:31/0 over -run.

SUGGESTED MOTION
HOOKS BRIDGE REPLACEMENT
Move that the Board of County Commissioners approve Change Order
No. 4 on the Hooks Bridge Replacement project. Change Order
Number 4 deletes the asphalt prime coat and adjusts the hot
bituminous pavement quantity to the as constructed quantity. The
contract price increases by $444.00. The new contract price
becomes $335,336.78

CHANGE ORDER
INSTRUCTIONS
A. GENERAL INFORMATION
This document was developed to provide a uniform format for handling contract changes
that affect Contract Price or Contract Time. Changes that have been initiated by a Work
Directive Change must be incorporated into a subsequent Change Order if they affect
Price or Time.
Changes that affect Contract Price or Contract Time should be promptly covered by a
Change Order. The practice of accumulating change order items to reduce the adminis-
trative burden may lead to unnecessary disputes.
For supplemental instructions and minor changes not involving a change in the Contract
Price or Contract Time, a Field Order may be used.
B. COMPLETING THE CHANGE ORDER FORM
Engineer initiates the form, including a description of the changes involved and attach-
ments based upon documents and proposals submitted by Contractor, or requests from
Owner, or both.
Once Engineer has completed and signed the form, all copies should be sent to Contractor
for approval. After approval by Contractor, all copies should be sent to Owner for
approval. Engineer should make distribution of executed copies after approval by Owner.
If a change only applies to price or to time, cross out the part of the tabulation that does
not apply.

2. Add note -to drawing sheet 4 of 12 to read:
"11. in- the event the Contractor cannot complete all paving
in accordance with the specifications prior to October
1, 1987, the typical pavement sections shall be
modified to include 3-1/2 of asphalt placed in a 2
bottom lift and a 1-1/2" top lift with a tack coat
between. The bottom lift shall be placed in accordance
with the specifications prior to October 15, 1987 and
the tack coat and top lift shall be placed in
accordance with the specifications in May 1988. The
actual quantity placed will be the basis for payment in
accordance with the specifications and the contract
unit price."
3. Revise paragraph 1 of Addendum No. 1, sentence nos. 3, 4, 5,
and 6 to read:
"The OWNER will obtain this final right-of-way by August 14,
1987. . The CONTRACTOR shall schedule the work to be
performed within these areas (outside of the existing
roadbed) to occur after August 14, 1987. The CONTRACTOR
will not be entitled to an extension of the Contract Time
due to this timing -of the final right-of-way acquisition.
If CONTRACTOR believes that a delay in OWNER'S furnishing
this right-of-way past August 14, 1987 entitles him to an
extension of the Contract time, Contractor may make a claim
therefore as provided in Article 12 of the General
Conditions of the Contract."
