RE: Connecticut General Account No. 0488282
Dear Danny,
Please amend Eagle County insurance as follows:
An Eagle County Government employee regularly scheduled
to work between 30-39 hours per week is eligible for,
insurance after satisfying the 30 day waiting period.
The employee shall have 30 days from the eligibility date
to submit a completed insurance enrollment form to the
County Personnel Department, who will forward the
completed form to Executives Service, Inc.- Eagle County
is responsible for the full premium due monthly for
part-time employees enrolled for coverage. Part-time
employees may be required to reimburse the County for a
portion of the premium as established by the County
Personnel policy.
If the part-time (30-39 hours/week) employee does not
elect insurance coverage within 30 days of eligibility,
and later desires insurance coverage, he/she will be
required to submit any required Proof of Insurability
forms. In this case, the employee (and dependents, if
applicable) will not become eligible for insurance
coverage until they are accepted by the underwriters of
Connecticut General Life Insurance Company. If an
employee is not approved by the underwriters of CG, the
employee (and dependents, if applicable) will be
ineligible for insurance coverage by the County.
Employees denied coverage will be able to re -apply for
coverage.
